Questionaire
Below is a questionnaire that has been put together the club Welfare Officer, it is designed to get feedback from the players to help the management team run the club to its optimum potential whist empowering the players to voice their opinions and help the club to pinpoint any areas where improvements can be made.
Print it off, fill it in (as much as you want to) be as honest as you can and hand it back into the Welfare Officer (Sam) in total confidence that your voice will be heard but more importantly listen to. |
